Sarah Bartlett Sweeney is the founder of Threshold & Containment Consulting, an experienced consultant working across mental health, learning disability, autism, and psychologically informed services. She is a Registered Nurse in Learning Disabilities and Autism with over 27 years of experience across mental health and learning disability healthcare leadership and commissioning, spanning frontline practice, leadership, service development, project management and organisational consultancy. Her work is grounded in a deep understanding of the emotional, ethical, and systemic realities of care provision alongside pragmatism with a deep working knowledge of interdependent systems. Sarah brings a strong psychodynamic and systems-based perspective, supporting organisations to understand how anxiety, responsibility, risk, and unconscious dynamics shape behaviour, culture, and decision-making — particularly under regulatory and emotional pressure. She has an extensive, authentic UK-wide professional network, enabling insight, collaboration, and influence across health and social care sectors. She has extensive experience in: - Mental health and learning disability service concept development, supported living and complex needs service set-up , independent mental health hospitals, clinical governance, safeguarding, and risk. Sarah holds a degree in Nursing and a first-class honours in Art & Design, which informs her capacity to think symbolically, creatively, and systemically about organisations and the people within them. She is currently undertaking an MSc in the Psychodynamics of Human Development, further deepening her work in systems-psychodynamic and evolving Tavistock-informed organisational consultancy.
